What You Need to Do to Get Paid

Online:

  1. Visit the official settlement website.
  2. Enter the Unique ID and PIN found on your settlement notice postcard.
  3. If you do not have a notice, contact the administrator at 1-888-835-0109 to see if you are eligible.

  • Download the Claim Form from the documents page.
  • Fill it out completely and sign it.

Class members who file a valid claim will receive a pro rata cash payment estimated to be up to $90. The final amount depends on the number of valid claims filed.
