You may be eligible for the General Physician, P.C. settlement if you:
- Reside in the United States
- Received a letter from General Physician, P.C. notifying you that your private information may have been accessed or acquired by an unauthorized party as a result of the data incident that occurred between April 6, 2024 and June 12, 2024 (the Class Period)
- Had any of the following types of information potentially exposed: full name, address, Social Security Number, financial account information, date of birth, medical history information, mental and physical treatment information, diagnosis information, treating physician, medical record number, health insurance information, policy number, subscriber number, or group/plan number

What You Need to Do to Get Paid
To get paid from the General Physician, P.C. settlement:
Claim Online
- Visit the official settlement claim site and go to the online claim form.
- Complete the form, selecting either Cash Payment A (documented losses up to $5,000) or Cash Payment B (estimated $60 flat payment).
- If claiming documented losses, upload supporting documentation.
- You may also elect two years of Credit and Medical Records Monitoring.
- Submit your claim online by May 27, 2026 at 11:59 p.m. ET.
Claim by Mail
- Download and print the claim form from the settlement website, or call the Settlement Administrator to request a paper copy.
- Complete the form, selecting your preferred payment option and attaching any required documentation.
- Mail your completed claim form to Kroll Settlement Administration LLC, postmarked no later than May 27, 2026.
Cash Payment A (up to $5,000) requires documented losses with reasonable supporting documentation. Cash Payment B ($60 estimated, subject to pro rata adjustment) requires no proof of specific losses. Credit and Medical Records Monitoring (2 years, value $179.40/year) available regardless of proof.
Settlement Payout
General Physician, P.C. settlement payments are between $60 and $5,000 per person, depending on which payment option you choose. Class members can elect Cash Payment A, which reimburses documented out-of-pocket losses related to the breach up to $5,000, or Cash Payment B, a flat estimated payment of $60 that may adjust up or down based on how many valid claims are filed. In addition to either cash payment, you can also claim two years of Credit and Medical Records Monitoring through CyEx Medical Shield, valued at an estimated $179.40 per year. Payments will be distributed after the court grants final approval of the settlement and any appeals are resolved.

Background of the Lawsuit
In October 2024, General Physician, P.C. (GPPC) announced that a cybersecurity incident had occurred between April 6, 2024 and June 12, 2024. The breach potentially exposed the private information of approximately 490,210 individuals, including names, addresses, Social Security Numbers, financial account information, dates of birth, medical history, mental and physical treatment records, diagnosis information, treating physician names, medical record numbers, health insurance details, policy numbers, subscriber numbers, and group/plan numbers.
Plaintiffs filed a lawsuit in the Supreme Court of the State of New York, County of Erie, captioned Newhart v. General Physician, P.C., Index No. 815961/2024, alleging that GPPC failed to adequately protect patients’ private information. GPPC denies all claims and maintains it did not do anything wrong.
Rather than proceed to trial, both sides agreed to a $2,500,000 settlement. The court has not ruled in favor of either party. A Final Approval Hearing is scheduled for June 4, 2026. For more information, visit the official settlement website.
