To get paid from the Legacy Health settlement:
Claim Online
Visit www.PixelLegacyHealthSettlement.com and use your Unique ID and Claim Login PIN (found on your settlement notice) to submit your claim online. This is the required method for electronic or ACH payments.

Download and complete the Claim Form from the settlement website. Mail it with your name, address, and Unique ID to:

Medical Shield Complete
All class members automatically receive one year of Medical Shield Complete identity protection service. No claim submission required. An enrollment code and link were emailed to you. If you did not receive the email, contact the Settlement Administrator at (833) 647-9039.
Legacy Health settlement payments consist of two benefits: (1) a Claim Form submission for a $15 cash payment, and (2) automatic enrollment in one year of Medical Shield Complete identity protection service at no cost.
The $15 cash payment requires you to submit a valid Claim Form by March 16, 2026, online or by mail. The Medical Shield Complete benefit—which includes credit monitoring, dark web monitoring, $1,000,000 identity theft insurance, and other protective features—is provided automatically to all class members without requiring a claim submission.
Katherine Layman filed a class action lawsuit against Legacy Health, alleging that the healthcare provider disclosed patients’ personally identifiable information to Meta Platforms Inc. (Facebook) and Alphabet Inc. (Google) without permission. The plaintiff asserted claims for breach of confidence, breach of implied contract, unjust enrichment, and negligence.
Legacy Health denied any wrongdoing. Rather than proceed to trial, both parties agreed to settle the case to avoid the costs, risks, and uncertainties of continued litigation. The settlement was preliminarily approved by the Circuit Court of Multnomah County on December 29, 2025. A Final Approval Hearing is scheduled for April 16, 2026, at 10:00 a.m. PT to determine whether the court will grant final approval.
