What It Means

Claim forms usually ask for identifying info and eligibility details (dates, account numbers, purchases, etc.). Some settlements require proof (receipts, statements, documentation). Others allow a sworn statement under penalty of perjury. If you submit incomplete or inaccurate info, your claim can be denied.
