  • You must have received a clinical adherence call from OptumRx, Inc. between April 20, 2020, and October 22, 2025.
  • The call must have been directed to a cellular telephone or VOIP service number.
  • The call must have used an artificial or prerecorded voice.
  • You must NOT have been an OptumRx, Inc. customer or account holder at the time.
  • Calls regarding prescription refill reminders or COVID-19 vaccines are NOT included.
