Who’s Eligible

  • All individuals residing in the United States who were sent a notice of the Data Incident that occurred on Group Health Cooperative of South Central Wisconsin’s computer systems on or about January 24, 2024.
  • All individuals residing in the United States whose personally identifiable information or private health information may have been impacted in the Data Incident.
  • Excluded: Directors, officers, and agents of Defendant; governmental entities; and the Judge assigned to the litigation and their staff.

What You Need to Do to Get Paid

You can submit a claim online or by mail: Visit the official settlement website at www.GHCSCWSettlement.com to file electronically. Alternatively, download the Claim Form or request one by phone, complete it in blue or black ink, and mail it to the Settlement Administrator. Ensure your claim is submitted online or postmarked by the deadline of January 20, 2026.

For Cash Payment A (Documented Losses), you must submit reasonable documentation such as phone bills, receipts, or account statements. For Cash Payment B or Medical Monitoring, you must provide the Unique ID from your notice or attest to your class membership.

Settlement Payout

Estimated Payout Per Person

Settlement Class Members may choose one of the following benefits:

  • Medical Data Monitoring: Three years of CyEx’s Medical Data Monitoring product, including credit monitoring, dark web scanning, and $1 million in identity theft insurance.
  • Cash Payment A (Documented Losses): Reimbursement for documented out-of-pocket losses reasonably related to the Data Incident, up to a maximum of $5,000.00. Documentation is required.
  • Cash Payment B (Alternate Cash Payment): An estimated pro rata cash payment of approximately $100.00. The final amount depends on the number of valid claims and remaining funds.

The lawsuit alleges that GHCSCW failed to prevent a cyberattack on its computer systems on or about January 24, 2024, which potentially compromised the personal and private health information of members.
