You may be eligible for the Blackstone Valley Community Health Care settlement if you:

What You Need to Do to Get Paid
To get paid from the Blackstone Valley Community Health Care settlement:
Claim Online
- Visit the official settlement claim site and go to the online claim form
- Log in using the Login ID and PIN from the notice mailed to you (if you can’t locate them, contact the Settlement Administrator to request your credentials)
- Select the benefits you’re claiming: credit monitoring, lost time reimbursement (up to $80), out-of-pocket expenses (up to $250), and/or identity theft/fraud losses (up to $2,500)
- Upload or describe any supporting documentation required for cash reimbursement claims
- Submit your completed claim form by June 1, 2026
Claim by Mail
- Download and print the claim form from the settlement website, or call the Settlement Administrator at (855) 256-5310 to request a paper copy
- Complete and sign the form, attaching any required supporting documents
- Mail the completed form to Simpluris, the Settlement Administrator, so it is postmarked no later than June 1, 2026
For Documented Ordinary Expenses (up to $250): Must provide proof like receipts showing out-of-pocket expenses for credit reports, monitoring, ID replacement, or postage. For Documented Extraordinary Expenses (up to $2,500): Must provide proof that theft/fraud was probably caused by the Data Incident and that you attempted to prevent the loss. For Lost Time (up to $80): No proof required, but must briefly describe the time spent.
Settlement Payout
Blackstone Valley Community Health Care settlement payments are between $80 and $2,500 depending on the type of claim you submit. Class members can claim up to $80 for lost time (up to four hours at $20 per hour), up to $250 for documented out-of-pocket expenses, and up to $2,500 for documented losses from identity theft or fraud linked to the data breach. All class members can also enroll in three years of credit monitoring through CyEx Identity Defense Total at no cost. Cash payments are subject to a $525,000 total cap; if claims exceed that amount, individual payments will be reduced pro rata. Payments will be distributed after the court grants final approval and any appeals are resolved. Payment options include PayPal, Venmo, Zelle, a virtual prepaid card, or a physical check.

Background of the Lawsuit
In November 2023, Blackstone Valley Community Health Care (BVCHC) was hit by a targeted cyberattack on its computer systems. Plaintiff Alba Peralta Perez filed suit alleging that certain files containing private information, potentially including names and Social Security numbers, were accessed during the attack.
The case, Perez v. Blackstone Valley Community Health Care, Inc., C.A. No. PC-2025-06050, is pending in the Superior Court of Providence County, Rhode Island. BVCHC denies any wrongdoing, and the court has not ruled on the merits of the claims.
Both sides agreed to settle to avoid the costs and uncertainty of continued litigation. The proposed settlement makes cash payments and credit monitoring available to affected individuals, subject to court approval at a final approval hearing scheduled for June 23, 2026.
