To get paid from the Eye Physicians of Central Florida settlement:
Claim Online
- Go to eyephysicianscentralflsettlement.com/form/claim
- Log in using the Unique ID and PIN from the notice mailed to you. If you can’t locate them, email info@EyePhysiciansCentralFLSettlement.com with your full name and mailing address to request your credentials.
- Complete the claim form, selecting the benefits you want: credit monitoring, documented ordinary losses (up to $2,000), and/or documented extraordinary losses (up to $7,500).
- Upload any required supporting documentation for loss claims.
- Submit your claim by May 15, 2026.
Claim by Mail
- Download the claim form at eyephysicianscentralflsettlement.com or call (888) 511-1472 to request a paper copy from Simpluris, the settlement administrator.
- Fill out the form completely, sign it, and attach any supporting documentation for loss claims.
- Mail the completed form to the settlement administrator. Your envelope must be postmarked no later than May 15, 2026.
Claimants can file for Documented Ordinary Losses (up to $2,000) and Documented Extraordinary Losses (up to $7,500) with supporting documentation such as credit card statements, bank statements, invoices, telephone records, and receipts. Self-prepared documents alone are insufficient. Credit Monitoring Services (2 years, $1M identity theft insurance) are available without proof of losses.
Settlement Payout
Eye Physicians of Central Florida settlement payments vary depending on the type of losses claimed. Class members can claim up to $2,000 for documented ordinary out-of-pocket losses, including up to three hours of lost time at $25 per hour. Those who suffered fraud or identity theft directly tied to the breach can claim up to $7,500 for documented extraordinary losses. All class members are also eligible to enroll in two years of credit monitoring services, which includes $1 million in identity theft insurance. Payment options include PayPal, Venmo, Zelle, virtual prepaid card, or physical check. Payments will be distributed after the court grants final approval and any appeals are resolved.

Background of the Lawsuit
In November 2023, a targeted cyberattack on Eye Physicians of Central Florida’s computer systems accessed files containing patients’ private information. The exposed data may have included names, addresses, dates of birth, medical diagnosis and treatment information, health insurance information, and financial information.
Plaintiff Alisa Connell filed a lawsuit in the Circuit Court for Orange County, Florida, alleging that Eye Physicians failed to adequately protect that information. The case, Connell v. Eye Physicians of Central Florida, P.L.C., Case No. 2023-CA-017660-O, seeks relief on behalf of all U.S. residents notified of the breach.
Eye Physicians denies any wrongdoing, and the court has not ruled on the merits. Both sides agreed to settle to avoid the costs and uncertainty of continued litigation. The settlement is pending final court approval.
