Who’s Eligible
- All living individuals residing in the United States who were sent a notice of the Data Incident indicating that their Private Information may have been impacted in the Data Incident.
Class Members excluded:
- all persons who are directors, officers, and agents of Veradigm;
- governmental entities;
- the Judge assigned to the Action, that Judge’s immediate family, and Court staff; and
- any Settlement Class Member who timely and validly requests to opt out from the Settlement

What You Need to Do to Get Paid
To get paid from the Veradigm settlement:
Claim Online: Submit your Claim Form electronically at www.veradigmdatasettlement.com by Tuesday, March 3, 2026, 11:59 p.m. CT. To receive electronic or ACH payment, you must file online.
Claim by Mail: Mail your completed Claim Form, including supporting documentation, by U.S. mail postmarked no later than Tuesday, March 3, 2026, to:

You must select one or both cash payment options (up to $5,000 for documented losses or $50 alternative) and may also claim two years of CyEx Medical Data Monitoring. If claiming documented losses, submit reasonable documentation such as receipts, phone records, or email correspondence supporting the losses.
For Cash Payment A (Documented Losses), claimants must submit reasonable documentation of losses, which means documentation contemporaneously generated or prepared by a third party or the settlement class member supporting a claim for expenses paid. Non-exhaustive examples include telephone records, email correspondence, or receipts. Personal certifications, declarations, or affidavits do not constitute proper documentation, though they may be included to provide clarification or context. Claimants will not be reimbursed for expenses already covered by another source, including credit monitoring or identity theft protection products.
Settlement Payout
Veradigm settlement payments are available to eligible class members in two forms.
Cash Payment A offers up to $5,000 for documented losses related to the data incident, subject to pro rata adjustment.
Cash Payment B provides an estimated $50 as an alternative.
All class members may also claim two years of CyEx Medical Data Monitoring, which includes credit monitoring, dark web scanning, identity theft insurance up to $1,000,000, and fraud resolution assistance.
Pro rata increases or decreases apply to cash payments based on total valid claims received.
- Attorneys’ Fees: up to $3,500,000
- Service Awards: up to $2,500 per Class Representative (4 representatives)
- Settlement Administration Costs: approximately $1.35 million (capped at $1.79 million)

Background of the Lawsuit
Veradigm is an Illinois-based healthcare technology company offering electronic health record and medical practice management software to healthcare providers. In December 2024, cybercriminals unlawfully accessed and acquired information for more than two million patients of Veradigm’s clients stored on Veradigm’s servers. The compromised private information included names, contact details, date of birth, health record data, insurance claim data, payment information, and other identifiers such as Social Security numbers and driver’s license copies. Not all data elements were present for every impacted individual.
The plaintiffs alleged that Veradigm failed to adequately protect patient data. Veradigm denies any wrongdoing whatsoever, and no court has made any judgment or determination that Veradigm acted improperly. Both parties agreed to this settlement to avoid the cost and risk of trial and appeals while providing benefits to affected class members.
The settlement establishes a $10,500,000 fund to compensate eligible class members. Settlement class members can claim up to $5,000 for documented losses related to the data incident, or choose an alternate $50 payment. All class members may also claim two years of medical data monitoring services from CyEx, including credit monitoring, dark web scanning, identity theft insurance, and fraud resolution assistance.
