What You Need to Do to Get Paid

Submit a Claim Form online at www.TrinityHealthClassClaim.com using your Claim Number if available. You may also mail a completed paper Claim Form to the Settlement Administrator. All claims must be postmarked or submitted online by January 19, 2026.

No proof is required for the pro-rata cash payment, but a claim form must be submitted. Reasonable documentation is required to claim reimbursement for out-of-pocket losses.

Who is included in the Settlement Class?
The Settlement Class includes all persons whose data may have been impacted by the Data Security Incident and to whom Trinity Health sent via direct mail notices of the Data Security Incident, that occurred on or about January 20, 2021, addressed to a California address.
The Settlement provides a Settlement Fund of $450,000. Class Members may receive a Pro-Rata Cash Payment estimated between $11 and $231, and reimbursement of up to $1,000 for Documented Out-of-Pocket Losses.

Yes, the Court appointed Patrick N. Keegan of Keegan & Baker LLP as Class Counsel to represent the Settlement Class. You will not be charged separately for their services.
