What You Need to Do to Get Paid

To get paid from the Signature Performance settlement:

Claim Online or by Mail

  1. Visit the settlement website at https://www.signaturedatabreachsettlement.com/ to download a Claim Form or request one by calling 1-888-822-4801
  2. Complete the Claim Form with your information and select your benefit option: documented loss reimbursement (up to $5,000 with proof), undocumented payment (approximately $200), California statutory payment if applicable (approximately $200), or medical data monitoring (three years)
  3. If claiming documented losses, attach supporting documentation of your unreimbursed ordinary losses incurred on or after January 17, 2024, such as credit monitoring costs, fraud fees, unauthorized charges, or transportation expenses
  4. Mail your completed Claim Form and documentation to the Settlement Administrator at P.O. Box 301132, Los Angeles, CA 90030-1132, or submit it online through the settlement website
  5. Submit your claim by May 7, 2026 (postmarked or submitted by this deadline)

For Cash Payment A (documented losses up to $5,000), you must submit supporting documentation of your actual, unreimbursed losses incurred on or after January 17, 2024. For Cash Payment B (undocumented payment of approximately $200), no documentation is required.

Signature Performance settlement payments are up to $5,000 for documented losses, or approximately $200 without documentation, plus up to $200 additional for California residents. All amounts are subject to pro rata adjustment based on total eligible claims. Payments are distributed by check after court approval and appeals are resolved, typically within several months to over a year. Medical data monitoring services are also available for three years.

Background of the Lawsuit

A cybersecurity incident occurred in or around January 2024 involving Signature Performance, Inc., in which an unauthorized third party may have gained access to files containing sensitive personal information, including personally identifiable information and protected health information. The incident affected individuals notified by Signature Performance, Inc., Adventist Health Tulare, Adventist Health System/West, and Southeastern Regional Medical Center d/b/a UNC Health Southeastern.

A class action lawsuit was filed by Curtis McClean and seven other plaintiffs asserting that Signature Performance is responsible for the data incident and that affected individuals are entitled to damages. Signature Performance denies any wrongdoing and denies liability but has agreed to settle the lawsuit on a class-wide basis to avoid the cost and risk of trial and appeals while providing benefits to settlement class members.
