To get paid from the Outcomes One settlement:
You can receive $75 without documentation, or up to $3,500 if you provide reasonable documentation of fraud or identity theft losses directly traceable to the data incident, such as bills, invoices, phone records, emails, or receipts.
Outcomes One settlement payments are up to $3,500 per person for documented fraud or identity theft losses, or $75 without documentation, plus one year of free medical data monitoring. Payments are distributed after Court approval becomes final. Amounts may be adjusted pro rata if total claims exceed or fall short of available settlement funds.
A class action lawsuit was filed against Outcomes One, Inc. following a data incident on July 1, 2025, in which an unauthorized third party gained access to an employee email account. The breach may have exposed settlement class members‘ private information, including names, demographic information, medical provider information, health insurance information, and medication information.
Outcomes One denies the legal claims and any wrongdoing or liability. The Court has not made any determination regarding wrongdoing. The parties agreed to settle to avoid the risks, costs, and uncertainty of continuing litigation. The settlement provides cash payments (either documented losses up to $3,500 or a flat $75), plus one year of medical data monitoring. For more information, visit the official settlement website.
