For documented out-of-pocket losses (up to $15,000), attach supporting documentation like receipts proving costs from fraud, identity theft, or credit monitoring. For pro rata cash payments, no documentation needed. All settlement class members automatically receive 2 years of credit monitoring with no claim required.
Claimants seeking documented out-of-pocket loss payments must submit supporting documentation including receipts or other non-self-prepared documents showing costs incurred. Self-prepared documents alone are insufficient. For pro rata cash payments, no documentation is required.
Numotion settlement payments include two cash payment options: documented out-of-pocket losses up to $15,000 per person with supporting documentation, or a pro rata share of remaining Settlement Fund money. All class members automatically receive 2 years of credit monitoring. Those with compromised Social Security numbers may claim medical monitoring.
Numotion, a mobility equipment company, experienced two significant data breaches. In March 2024, unauthorized access occurred between February 29 and March 2, affecting records of approximately 685,264 customers and employees containing personal information. In September 2024, a phishing attack resulted in unauthorized access to user accounts, potentially compromising personal health information of approximately 494,326 individuals.
Multiple class action lawsuits were filed across several states. Numotion denies liability and wrongdoing. The parties reached a settlement to avoid litigation costs and risks. Under the settlement, Numotion will pay $4 million to resolve all claims related to both data incidents.
