What You Need to Do to Get Paid

To get paid from the Balance Autism settlement:

Claim Online

  1. Visit the official Balance Autism settlement claim site
  2. Complete the online claim form with your personal information
  3. If you received a notice with a Unique ID, enter it on your form
  4. Select the benefit you want to claim: the $50 alternative cash payment, reimbursement for ordinary losses (up to $400), reimbursement for extraordinary losses (up to $4,000), lost time compensation (up to $80), or two years of free credit monitoring
  5. For loss reimbursement claims, upload supporting third-party documentation
  6. Submit your claim online no later than June 1, 2026

Claim by Mail

  1. Download and print the claim form from the settlement website, or call the Settlement Administrator at 833-908-9340 to request one by mail
  2. Fill out the form completely in blue or black ink
  3. Attach any required supporting documentation for loss reimbursement claims
  4. Mail your completed form to Analytics Consulting LLC, the Settlement Administrator
  5. Your envelope must be postmarked no later than June 1, 2026

Credit Monitoring and Alternative Cash Payment require no documentation. Lost Time requires attestation under penalty of perjury describing activities. Ordinary Losses require third-party documentation (receipts, bills). Extraordinary Losses require third-party documentation proving fraud/identity theft causation; self-prepared documents alone insufficient.

Estimated Payout Per Person

Balance Autism settlement payments are up to $4,000 per person for documented extraordinary losses such as identity theft or fraud directly linked to the data breach. Class members who experienced ordinary out-of-pocket expenses can claim up to $400, and those who spent time dealing with the breach can claim up to four hours at $20 per hour, capped at $80. If you’d rather skip the documentation, there’s a flat $50 cash payment option available instead. Credit monitoring alone requires no documentation and is available to all eligible claimants. Payments will be issued after the court grants final approval, with the Final Approval Hearing scheduled for June 12, 2026. Mail claimants receive a physical check; online claimants can opt for electronic payment.
