What You Need to Do to Get Paid
To get paid from the Asheville Eye Associates settlement:
Claim Online
Visit www.AEADataSettlement.com and submit your claim form online by April 6, 2026. For reimbursement of documented losses up to $1,250, attach supporting documentation showing the expenses were caused by the data incident.
Claim by Mail
Mail your completed claim form with supporting documentation to Asheville Eye Associates Data Incident Litigation, c/o Kroll Settlement Administration LLC, PO Box 225391, New York, NY 10150-5391. Your claim must be postmarked by April 6, 2026.
All class members automatically receive a $10 voucher and one year of identity theft protection without filing a claim.
To receive reimbursement for documented losses, you must submit reasonable supporting documentation for each claimed out-of-pocket expense. Documented losses include unreimbursed bank fees, card reissuance fees, overdraft fees, late fees, over-limit fees, charges from banks or credit card companies, interest on payday loans, and costs of credit monitoring or identity theft insurance purchased in response to the data incident. Self-prepared documents alone are insufficient but can support other documentation. Without supporting documentation, your claim will be denied.

Asheville Eye Associates settlement payments include automatic benefits plus optional reimbursement. All class members automatically receive a $10 voucher for eyeglasses and one year of identity theft protection. You can claim up to $1,250 in reimbursement for documented out-of-pocket expenses caused by the data breach if you submit a claim form with supporting documentation by April 6, 2026.

In November 2024, Asheville Eye Associates was the victim of a cyberattack in which hackers compromised the personally identifiable information and protected health information of current and former patients. The exposed information included names, addresses, health insurance information, Social Security numbers, and medical treatment details. Asheville Eye Associates began notifying affected individuals on February 4, 2025.
