Who’s Eligible
- You are included in the Settlement Class if you are a living individual in the United States whose Private Information was implicated in the Data Incident.
- The Data Incident was discovered on or about March 5, 2024, by Affiliated Dermatologists & Dermatologic Surgeons, P.A.
- Excluded from the Settlement Class are directors and officers of the Defendant, governmental entities, the Judge assigned to the lawsuit, the Judge’s immediate family, and Court staff.

What You Need to Do to Get Paid
You can file a claim in two ways:
1. File Online
- Visit www.AffiliatedDDSDBSettlement.com.
- Submit your Claim Form online by February 15, 2026.
2. File by Mail
- Download the Claim Form from the settlement website.
- Mail the completed form to the Settlement Administrator at PO Box 2684, Portland, OR 97208-2684.
- Ensure it is postmarked by February 15, 2026.
Documentation is required for Cash Payment A (Documented Losses) to prove expenses. No documentation is required for Cash Payment B ($40 Alternate Cash).
Settlement Payout
Settlement Class Members may select one of the following benefits:
- Cash Payment A (Documented Losses): Reimbursement of up to $5,000 for documented out-of-pocket losses reasonably related to the Data Incident (e.g., bank fees, communication charges, credit monitoring costs).
- Cash Payment B (Alternate Cash): A cash payment of $40 (subject to pro rata reduction if claims exceed the $1 million cap). No documentation is required.
- Credit Monitoring: All Class Members automatically receive three years of free Credit Monitoring services (no claim form needed).
- Cash Payment Cap: $1,000,000 (payments reduced pro rata if exceeded)
- Attorneys’ Fees & Costs: Up to $815,000 (paid separately)
- Service Awards: Up to $2,000 per Class Representative
