  • You are included in the Settlement Class if you are a living individual in the United States whose Private Information was implicated in the Data Incident involving Regional Obstetrical Consultants, P.C. on or about May 6, 2024.
  • Private Information includes names, dates of birth, addresses, phone numbers, medical record numbers, insurance ID numbers, diagnosis, medical history, procedures, and other personally identifiable or protected health information.
  • Excluded are directors and officers of the Defendant, governmental entities, the presiding Judge and their staff/family, and individuals who validly opt-out.

What You Need to Do to Get Paid

You can submit a Claim Form online at the official settlement website or by mail. The deadline to file is February 15, 2026. If mailing, the form must be postmarked by this date.

Documentation is required for Cash Payment A (Extraordinary Losses) and Cash Payment B (Ordinary Losses). No documentation is required for the $50 Alternate Cash Payment.

Class members can submit a claim for one of the following benefits:

  • Cash Payment A (Extraordinary Documented Losses): Reimbursement of up to $7,500 for documented monetary losses arising from identity theft or fraud fairly traceable to the data incident.
  • Cash Payment B (Ordinary Documented Losses): Reimbursement of up to $2,000 for documented out-of-pocket expenses such as credit freeze costs, bank fees, or communication charges.
  • Cash Payment C (Alternate Cash Payment): A flat cash payment of $50 without the need to provide documentation.
