What You Need to Do to Get Paid

Claims can be filed online at the official settlement website by February 9, 2026. Alternatively, you may mail a completed Claim Form to the Settlement Administrator at PO Box 64053, Saint Paul, MN 55164. Claims must be postmarked no later than February 9, 2026.

No proof is required for the estimated $75 cash payment or credit monitoring. Documented proof is required for out-of-pocket expense reimbursement claims up to $5,000.

Settlement Class Members may submit a claim for an estimated $75 cash payment (pro rata) OR reimbursement for documented out-of-pocket losses up to $5,000. Eligible expenses include bank fees, communication charges, postage, and costs related to identity theft. Valid claimants may also receive 2 years of credit monitoring and identity theft protection.

The Litigation arises out of a cybersecurity incident involving Mystic Valley Elder Services’ network on or around April 5, 2024. Plaintiffs allege unauthorized access to files containing personal information. Mystic Valley denies wrongdoing and liability but agreed to the Settlement to resolve the matter.
