Individuals residing in the United States whose private information was potentially exposed in the Liberty Hospital data incident on December 19, 2023.
Eligible States
Claims for documented expenses must be supported by reasonable third-party documentation such as receipts or invoices. Self-prepared documents are insufficient. The alternative cash payment option does not require documentation.
Settlement Class Members may submit a claim for Documented Expenses and Losses up to $500.00 for unreimbursed costs fairly traceable to the Data Incident. Alternatively, Class Members may choose an Alternative Cash Payment estimated to be $150.00, subject to pro rata adjustment.
The lawsuit concerns a data security incident that occurred on or about December 19, 2023, in which an unauthorized threat actor accessed the Defendant’s computer network and potentially sensitive information. The Defendant denies all claims of wrongdoing but agreed to the Settlement to avoid the costs and risks of a trial.
