  • Living individuals in the United States who were sent a notice by Falcon Healthcare that their Private Information may have been impacted in the Data Incident.
  • The Data Incident is the cybersecurity incident that took place between April 29, 2022, and July 3, 2022.
  • Excluded are directors and officers of Falcon Healthcare, the Judge assigned to the Action, that Judge’s immediate family, and Court staff.

What You Need to Do to Get Paid

You must complete and submit a Claim Form online at www.FalconDataSettlement.com or by mail postmarked no later than January 26, 2026. You must select either Cash Payment A (Documented Losses) or Cash Payment B (Alternative Cash Payment), and may also select Medical Data Monitoring.

No documentation is required for the estimated $100 Alternative Cash Payment. Reasonable documentation (such as telephone records, correspondence, emails, or receipts) is required to claim up to $5,000 for Documented Losses.

The Plaintiff alleges that Falcon Healthcare is liable for a cybersecurity incident that occurred between April 29, 2022, and July 3, 2022, where Private Information related to certain patients was accessed and downloaded. Falcon Healthcare denies all wrongdoing and liability but agreed to the Settlement to avoid the risks and costs of litigation.
