What You Need to Do to Get Paid
Submit a claim form online at the official settlement website by March 5, 2026. Alternatively, you can download, print, and mail a paper claim form to the Settlement Administrator, postmarked by the deadline.
Documentation is required for out-of-pocket loss claims (e.g., receipts, phone bills, bank statements). Lost time claims and credit monitoring do not require documentation.
Settlement Payout
Class members can submit a valid claim to receive the following benefits:
- Monetary Losses: Reimbursement of up to $3,000 for documented out-of-pocket losses fairly traceable to the Data Incident.
- Lost Time: Payment for up to 4 hours of lost time at $15 per hour (up to $60 total). This amount is included in the $3,000 cap.
- Credit Monitoring: 2 years of free single-bureau credit monitoring and identity protection services.
- Settlement Fund: $1,500,000
- Attorneys’ Fees & Costs: Up to $400,000
- Service Awards: Up to $5,000 ($2,500 per Representative)
- Administration Costs: Deducted from the Settlement Fund
