Eligible States
Submit a claim form online at the official settlement website by March 5, 2026. Alternatively, you can download, print, and mail a paper claim form to the Settlement Administrator, postmarked by the deadline.
Documentation is required for out-of-pocket loss claims (e.g., receipts, phone bills, bank statements). Lost time claims and credit monitoring do not require documentation.
