Claim Online

Claim by Mail
Download and complete the claim form from the settlement website. Mail it with any supporting documentation to: Pacheco, et al. v. CFMC, c/o Kroll Settlement Administration, PO Box 5324, New York, NY 10150-5324. Your claim must be postmarked by April 2, 2026.
You must submit a claim to receive any payment or credit monitoring services. Choose one benefit: credit and medical monitoring, alternative cash payment (estimated $40), or reimbursement for documented out-of-pocket losses up to $5,000.
To receive reimbursement for out-of-pocket losses, you must provide supporting documentation such as statements, receipts, or bills showing the charges. For alternative cash payment of approximately $40, no proof of loss is required.
Community First Medical Center settlement payments include one year of free credit and medical monitoring with identity theft insurance of $1,000,000 (no deductible), or an alternative cash payment estimated at $40, or reimbursement for documented out-of-pocket losses up to $5,000.
A data incident occurred on Community First Medical Center’s network systems on or about July 12, 2023, potentially compromising the personally identifiable information and personal health information of patients and individuals. The lawsuit, Pacheco, et al. v. Community First Healthcare of Illinois, Inc. d/b/a Community First Medical Center, alleged that the defendant was responsible for failing to prevent the data incident and asserted claims including negligence, negligence per se, breach of fiduciary duty, breach of contract, breach of implied contract, unjust enrichment, and violations of the Illinois Consumer Fraud and Deceptive Business Practices Act.
The defendant has denied all allegations and continues to deny all claims of wrongdoing or liability. Rather than proceed to trial, the parties reached a proposed settlement that allows class members to obtain compensation and credit and medical monitoring services without further delay. The settlement does not mean the defendant did anything wrong.
