What You Need to Do to Get Paid

Online: Visit the official website and click ‘Submit Claim’. You will need to upload digital copies of your balance bill.

Mail: Download the paper claim form from the website. Fill it out and mail it along with your proof to: Cigna LocalPlus Settlement Administrator 1650 Arch Street, Suite 2210 Philadelphia, PA 19103

You must provide proof of a ‘balance bill’ from your medical provider. This bill must show the difference between what the provider charged and what Cigna paid. You must also show proof you paid it or sign a statement saying you will use the money to pay it.

You may receive up to the full amount of your balance bill. If the total valid claims exceed the $300,000 cash fund, your payment will be a proportional share (pro rata) of the fund.

Background of the Lawsuit

The lawsuit alleges that Cigna incorrectly identified certain out-of-network doctors as ‘in-network’ for members of its LocalPlus plans. Because of this error, Cigna allegedly paid the doctors less than they expected, and the doctors then sent ‘balance bills’ to the patients for the difference. The plaintiffs claim this violated federal laws governing employee benefits.
