U.S. residents who received a written notification from Precision Imaging Centers regarding the data breach around June 2023.
Eligible States
To receive benefits, you must submit a Claim Form by January 31, 2026. You can submit the form online at the settlement website or by mail. If claiming reimbursement for expenses, you must attach supporting documentation such as receipts or account statements.
You must provide proof of class membership (Unique ID or name/address) and documentation for any expense reimbursement claims (e.g., bills, receipts, bank statements).
The settlement provides free identity protection and credit monitoring services. Additionally, class members may file claims for reimbursement of losses:
Ordinary Expense Reimbursement (Up to $500): For out-of-pocket costs like credit freezing, postage, and up to 4 hours of lost time at $20/hour.
Extraordinary Expense Reimbursement (Up to $5,000): For unreimbursed losses fairly traceable to the data breach (e.g., identity theft), including up to 8 additional hours of lost time at $20/hour.
The lawsuit alleges that Precision Imaging Centers failed to adequately protect patient data during a cybersecurity event on or around November 2, 2022. The compromised information included names, Social Security numbers, driver’s license numbers, and medical information. Precision denies any wrongdoing but agreed to settle to avoid litigation costs.
