Visit the claim form portal at https://carespringdatasettlement.com/form/claim and log in with your Unique ID and PIN from your settlement notice. Select your benefits: credit monitoring (automatic for all claimants), and either up to $4,500 for documented out-of-pocket losses or a $50 cash payment. Submit by April 16, 2026.

Claimants can receive $50 without proof of losses. To claim up to $4,500 in out-of-pocket losses, you must submit documentation showing the loss is fairly traceable to the October 2023 data incident and occurred between the incident date and the claim deadline. You must also show you made reasonable efforts to seek reimbursement from credit monitoring or identity theft insurance.
Carespring Health Care Management settlement payments include two years of free credit monitoring with $1,000,000 in identity fraud insurance for all class members. Additionally, you can claim either up to $4,500 for documented out-of-pocket losses traceable to the data breach, or an alternative $50 cash payment.
Plaintiffs alleged that Carespring Health Care Management suffered a targeted cyberattack in October 2023 that resulted in unauthorized access to protected health information and personally identifiable information. The lawsuit claimed Carespring failed to adequately protect class members’ information and asserted claims including negligence, breach of implied contract, and unjust enrichment.
Carespring denied the allegations and wrongdoing. The parties reached a settlement to resolve the matter without the expense and uncertainty of continued litigation. The settlement requires Carespring to provide free credit monitoring with identity fraud insurance and compensation for documented losses or alternative cash payments to class members who submit valid claims.
