What You Need to Do to Get Paid

To submit a claim, visit the official settlement website and complete the online Claim Form using the unique Login and Password found on the Postcard Notice sent to you. Alternatively, call the toll-free number to request a paper form and mail it to the Claims Administrator.

Claims for Ordinary or Extraordinary Losses require supporting documentation such as receipts, account statements, or bills. Claims for Lost Time require an attestation.

The settlement provides reimbursement for Documented Ordinary Losses up to $500, Lost Time up to $60 ($20/hour for 3 hours), and Documented Extraordinary Losses up to $1,500. Additionally, all class members are eligible for 1 year of 3B credit monitoring with $1 million in fraud protection.
